LEX 4 RETURNING STUDENTS
Students who completed the year at SMS or Sandhills Intermediate need to complete the 2010-11 Returning Student Registration Packet. This includes an updated demographic form, proof of residence in Lexington 4, and a 2010-11 Transportation Declaration.
LEX 4 NEW STUDENTS
New students to Lexington 4 will need to complete the entire 2010-11 Registration Packet. These forms can be picked up in the SMS office during regular office hours. To register you will need proof of residence in Lexington 4, an up-to-date shot record, a copy of the child's birth certificate, a copy of the child's social security card, and student records from the previously attended school.
Lexington 4 Proof of Residence: Please provide 2 proofs of residence - one from Category A and one from Category B.
Category A:
- Tax receipt showing that both property and dwelling are in Lexington District Four attendance area
- Mortgage statement or receipt
- Rental or lease agreement showing the address of the dwelling; the landlord's name, address, and telephone number. Rent receipts will not be accepted.
Category B:
- Cable television bill
- Telephone bill
- Utility Bill (electric/gas or water/sewer)
- House insurance invoice
- Bank statement
- Medicaid information
